Refactoring of ExercismExerciseGenerator (#586)

* Refactoring of ExercismExerciseGenerator responsible for generating test cases from problem specifications.
- added pragmas with test case metadata
- otherwise result should be same as original implementation

* Fix in method name generation and use of prefix

* Added unit test verifying exercise test generator
