Workout form: "+ Exercise" button

In the workout/workout_offering form, include a button like + Exercise, which lets the user create an "exercise stub" of sorts and place it in the workout at the appropriate position. They can go and "fill it in" later, kind of like how Canvas does quick assignment creation.

Need to discuss what these exercise stubs should look like.
